Buttock Implants

BUTT IMPLANTS – BBL

This information is provided by a plastic surgeon who has been an expert in buttock surgery for 20 years, trained by the best Brazilian plastic surgeons who truly pioneered and developed all surgical methods such as the Brazilian Butt Lift BBL. This information cannot answer all your questions, as each individual case is unique. Consult your plastic surgeon on every point, as nothing can ever replace a specialized consultation in aesthetic surgery.

Dr. Ouakil, plastic surgeon in Paris, provides information, pricing, photos, and a blog on buttock surgery, butt implants, buttock lipofilling (augmentation through fat injection into the buttocks), Brazilian Butt Lift BBL, and hyaluronic acid injection (Macrolane, Hyacorp) for the buttocks without surgery.

Indication: Who can be offered butt implants?

Butt implants are intended for individuals seeking an overall increase in volume, particularly for buttocks considered too flat and lacking "roundness," especially in profile. The implant, for safety and stability reasons, will be placed strictly within the gluteal muscle.

They do not alter the position of the infragluteal fold, meaning they do not lift the lower part of the buttocks. Their goal is to round and increase the volume of the central area (upper 2/3) of the buttock, without being able to increase the volume of the entire buttock, particularly at the lateral hollows. Lipofilling of the sides helps fill these hollows for a true BBL.

The results are very satisfactory as they are visible and natural. The maximum volume of the prosthesis is limited by the size of the gluteus maximus muscle, ranging from 220 to 500 g.

Principle of the butt implant

  • Cohesive silicone prostheses, soft and durable, with a smooth surface.
  • Risk of gel leakage is almost non-existent, lower than that of breast implants.
  • No need for routine replacement.
  • The combination with lipofilling (liposuction + reinjection) allows for a harmonious reshaping of the contour and provides round and natural buttocks.

2 hours of surgery and 2 days of hospitalization

  • Surgery under general anesthesia.
  • Single incision of 6–8 cm in the intergluteal fold.
  • Implants placed in or under the gluteus maximus muscle (upper 2/3 of the buttock), thus minimally perceptible visually and to the touch.
  • Volume limited by the size of the gluteus maximus muscle.

Post-operative care for butt implants

  • Difficulty walking and sitting during the first week → remain standing, on the stomach, or on the side.
  • Strict hygiene of the scar to prevent infections.
  • Severe pain but controlled by strong analgesics.
  • Possible persistent pain (pulling, hypersensitivity) for several weeks.
  • Minimum work stoppage of 15 days.
  • Return to sports involving the buttocks after 2 months.
  • No driving for 3 weeks.
  • Scar hidden in the intergluteal fold, invisible when standing.

Disadvantages of butt implants

Complications arise from improper positioning of the implant or its displacement. A subcutaneous implant (outside the muscle) should be avoided: guaranteed catastrophic result.

Rare but possible complications:

  • Sharp pain from contact with the sciatic nerve → reoperation necessary.
  • Implant visible under the skin and palpable.
  • Repeated inflammations with serous effusions.
  • Infections at the surgical site (fever, pain, discharge).
  • Effusions (hematomas, seromas) sometimes requiring drainage.
  • Possibility of definitive removal of the implants.

Conclusion on butt implants

As the reference technique for buttock augmentation, implants require perfect surgical mastery and reasonable expectations. It is a major operation with serious follow-up.

The combination with lipofilling (autologous fat) allows for a complete reshaping of the buttocks and hips, offering the best aesthetic results (composite BBL).
