Genioplasty Information

Genioplasty is the aesthetic surgery of the chin. Its goal is to modify its contour to recreate a harmonious face from both the front and profile views, particularly by increasing its projection from the side. We will also relate this to double chin surgery, which involves submental liposuction.
In a broader vision of the face (profiloplasty), we can also correct the lips or nose, for example.
We will not discuss genioplasties in the context of orthognathic surgery, which involves osteotomies (sectioning and repositioning of the maxillary and mandibular bone structures) to radically correct a poor position of the entire jaw. This heavy and complex surgery is performed in close collaboration with an orthodontist and requires a very long preparation and rehabilitation time.

Who is Genioplasty For and Why?

It is aimed at receding chins (retrognathia, retrogenia, microgenia, class 2 occlusion) or simply those that are too small: the chin is insufficiently developed and not advanced enough from the side, creating an imbalance with other facial structures.
In men, genioplasty can also be performed to enhance masculinity and the masculine expression of the face, even on a chin "considered normal in size." Chin augmentation using implants is the aesthetic surgery offered in these cases.

Double Chin: Clarifications.

Several factors contribute to this:

  • An underdeveloped chin with a short neck that does not adequately support the skin of the neck
  • Skin laxity related to aging
  • A fatty accumulation under the chin sometimes related to overall overweight
  • A marked relief of the muscles and submandibular glands

Liposuction or submental cryolipolysis will be used alone or in combination with chin advancement through genioplasty to eliminate this double chin. In some cases, particularly related to age, a facelift may be necessary. Unauthorized local injections in France can dissolve the submental fat without surgery.

What Implants are Used for Genioplasty?

There are two main types of chin prostheses:

  • Anatomical implants that cover the entire anterior part of the mandible, with a central body at the tip of the chin and lateral wings to widen a chin that is too narrow from the front. This is the preferred implant for masculinizing the mandible in men.
  • Tip implants that do not enhance the width from the front and primarily serve to project the tip of the chin from the side. This is the preferred implant for women with a receding chin or for double chin.

The prostheses are most often made of polymerized hard silicone, others in Medpor®. They are very well tolerated and do not require replacement.

What Type of Anesthesia is Used During Genioplasty?

Genioplasty is easily performed under simple local anesthesia, but may sometimes benefit from deep sedation (neuroleptanalgesia) for better comfort in the case of anatomical prosthesis placement or for anxious patients.
In all cases, genioplasty will be performed on an outpatient basis (no overnight stay at the clinic).

Where are the Scars Located During Genioplasty?

The incision is most often hidden inside the mouth at the back of the mucous part of the lower lip (invisible scar) or very discreetly under the chin.
A custom pocket is created, in contact with the mandibular bone, avoiding detaching the muscles of the chin too low and respecting the mandibular nerves.
The incision is closed with absorbable stitches.
The procedure lasts 30 minutes to 1 hour depending on the type of implant.

What are the Aftereffects of Genioplasty with Chin Prosthesis?

A compression dressing is kept for 5 days to prevent any displacement of the prosthesis.
A blended diet for 48 hours, antiseptic mouth rinses 3 to 4 times/day for 8 days. Avoid hot foods for the first 48 hours.
The result is immediate but slightly over-corrected by swelling for a few days. The prosthesis is invisible, thanks to the thickness of the muscles and fat of the chin.
Sensitivity disturbances of the lower lip are possible but almost always reversible. Infection is rare.
As with any implant procedure, poor positioning may require revision. The face and thus the bony chin are inevitably asymmetrical before and after.
Smoking is a relative contraindication as it weakens the mucosa and can cause infection.

What Happens to a Chin Implant?

The solid silicone implant is permanent and does not need to be replaced.
Sometimes a slight erosion of the mandibular bone at the contact with the implant is noted. This occurs slowly and progressively, without long-term consequences.
